Compassionate Care and Support for Seniors
Our team of geriatric specialists helps you better manage your changing health needs as you age. We offer comprehensive, interdisciplinary primary care and consultations to support patients and families coping with challenging conditions related to aging. The services we offer are:
- Primary care for adults 80 years and older
- Primary care for adults 65 years and older who have neurologic conditions like dementia, cognitive impairment, or neuro-degenerative disease
- Consultative visits to support care for adults 65 years and older who already have a primary care doctor

Dementia Care Program
The Dementia Care Program is a support program for patients of Senior Care clinic who have a diagnosis of dementia. The program is composed of a Geriatrician, Dr. Silvia Tee, Dementia Care Specialist, Michelle Melendez, NP, and Licensed Clinical Social Workers Beth St. John and Sarah Phoenix.
The Stanford Senior Care Dementia Care Program provides specialized dementia care management and partners with your primary care physician (PCP)/geriatrician to address dementia-related medical and psychosocial issues.
Your entry into the program begins with a referral from your Stanford Senior Care PCP, answering questionnaires, and a 90-minute clinic visit with Dementia Care specialist, Michelle Melendez, NP. The evaluation includes a review of your medical history, tests of memory and language, and standardized questionnaires that help us get to know you and your needs better. We then create a personalized care plan and follow-up plan in collaboration with your PCP.
Caregivers play a crucial role in supporting patients with dementia. Our Dementia Care Program provides a Caregiver Support Group which caregivers can join free of charge. It is a twice-monthly ongoing group currently meeting via Zoom and facilitated by our two social workers. The group is providing caregiver support and facilitates the exchange of ideas and tips for caregiving of loved ones with dementia. Topics discussed in the Dementia Caregiver Support Group include:
- Managing dementia-related behaviors
- Mood and dementia
- Navigating changing relationships
- Caregiver self-care
- When and how to ask for/access help
- Supporting patient quality of life
- Communication and dementia
